If your service has collected personal details and data and can provide it in:
- an exported format (eg. xls, csv)
- an other uploadable form (eg. pdf, doc)
You can skip to Part 2 of the Form and upload the personal details and data.
Otherwise, please enter the applicant details and data manually in Part 1.
